Why trichology starts with diagnosis
ParamedThe commonest mistake is to start with procedures. Mesotherapy and PRP work well where the follicle is alive and undernourished, and do not work at all when the cause is systemic: iron and ferritin deficiency, hypothyroidism, vitamin D deficiency, medication, rapid weight loss or a recent illness. In telogen effluvium that began 2–3 months after a stressor or COVID, hair often recovers on its own — and a course of injections there simply coincides with natural recovery. In androgenetic alopecia the opposite holds: without systemic therapy, injections give a temporary effect. So the appointment starts with trichoscopy and a conversation about blood tests, and the course is prescribed against an established cause.

How the appointment goes
ParamedFirst the history: when it started, whether anything happened 2–3 months before it started, how you eat, what medication you take, whether your parents lost hair, how pregnancy or menopause went. Then trichoscopy: under magnification you can see hair calibre and its variability, the state of the follicular openings, signs of inflammation and of miniaturisation — and it is miniaturisation that separates androgenetic alopecia from telogen effluvium, which the naked eye cannot do. Where needed the doctor orders blood tests: ferritin, full blood count, TSH, vitamin D, and for women a hormone panel. The plan follows the results: what gets treated systemically, and what gets added as procedures.

Preparation & aftercare
Paramed- Do not wash your hair on the day — trichoscopy reads better on unwashed hair
- No oils, serums or styling products on the day of the examination
- Bring blood test results from the last 6 months if you have them
- Think back to what was happening 2–3 months before the shedding began
- Do not colour your hair for 3–5 days before the appointment
- Do not wash your hair for 24 hours after injections
- No sauna, pool or hard exercise for a day
- Do not brush the area aggressively in the first few days
- Take the prescribed medication — without it the procedures deliver less
- Come back for repeat trichoscopy at 3–6 months for comparison

? How many hairs a day is normal? ⌄
Up to 100 a day is considered normal, and judging by that number is pointless: what matters is the trend and whether what sheds is replaced. The warning signs are hair coming out in clumps, a widening part, scalp showing through, and a change in hair calibre.
? Do I need blood tests? ⌄
Often yes, and it is not a formality. Iron deficiency without anaemia, hypothyroidism and vitamin D deficiency are the commonest reversible causes of shedding in women. Treating those with scalp injections is futile: until the cause is addressed, the hair will shed again.
? Will mesotherapy help with balding? ⌄
In androgenetic alopecia mesotherapy improves the quality of the hair you still have but does not stop miniaturisation — that needs systemic therapy. In telogen effluvium and diffuse thinning it works better. Which of those you have is what trichoscopy shows.
? Which is more effective — PRP, polynucleotides or exosomes? ⌄
They solve different problems and are often combined. PRP is the affordable baseline using your own growth factors. Polynucleotides work on scalp quality and microcirculation. Exosomes are the most expensive and the strongest signalling stimulus, usually after microneedling. The choice depends on cause, stage and budget — the doctor will explain the difference at the appointment.
? When will I see results? ⌄
Shedding stops at 6–8 weeks, new growth is visible by month 3–4, and the honest verdict comes at 6 months by comparing trichoscopy images. If someone promises results in a month, that is a promise about hair that was going to stay anyway.
? Do you treat alopecia areata? ⌄
We diagnose it; alopecia areata is an autoimmune condition and is managed by a dermatologist rather than a cosmetologist. The clinic has both a dermatologist and phototherapy, so in some cases treatment can happen here. If systemic immunosuppression is needed, you will be referred on.

ParamedHair grows about a centimetre a month and no procedure speeds that up — which is why there are no fast results in trichology. The first thing correct treatment does is stop the shedding; that shows at 6–8 weeks. New growth becomes visible by month 3–4, and the result can be judged honestly at 6 months by comparing trichoscopy images. A course of injections is usually 4–8 sessions 1–2 weeks apart, then maintenance every 1–2 months. In androgenetic alopecia treatment is lifelong maintenance: stop it and the process returns — better to know that before starting than a year in.
