How the appointment goes
ParamedThe specialist examines the foot and nail plates, and takes a sample if fungus is suspected — treating onychomycosis on appearance alone is pointless, because nail trauma, nail psoriasis and onychogryphosis all look similar. The foot is then softened and the work is done with a rotary instrument: hyperkeratosis is taken down in layers, a core callus is removed along with its core, a thickened plate is thinned. For an ingrown nail a tamponade or brace is fitted — it gently unrolls the edges of the plate over a few weeks, and it does not hurt. For fungus the ClearChoice laser programme is added. The appointment takes 30 to 60 minutes and you can walk out on it.
The tools

Preparation & aftercare

? How is a podiatrist different from a pedicurist? ⌄
A pedicurist works on appearance and is not licensed to treat. A podiatrist works on a medical problem: fitting correction systems for ingrowth, removing the core of a callus, managing fungus with lab confirmation, assessing the diabetic foot. Instruments are sterilised to a medical protocol.
? Does ingrown nail treatment hurt? ⌄
Fitting a tamponade or a brace is usually painless — the plate is not removed and nothing is cut. If the ingrowth is advanced and inflamed, the inflammation is settled first. Full surgical removal of the plate is a last resort in podology, and in most cases it can be avoided.
? Will the laser definitely cure nail fungus? ⌄
The honest answer: not always on its own. The laser suppresses fungus in the nail bed and works well when up to 50–70 % of the plate is involved. With total involvement or frequent recurrence, systemic therapy prescribed by a dermatologist is needed. ? Can I see a podiatrist if I have diabetes? ⌄
Not only can you — you should: self-treating the foot with diabetes is risky. Rotary instrument care with sterile burrs avoids the risk of cuts. Do tell us about the diagnosis; if it is poorly controlled, we will want sign-off from your endocrinologist.
? When will the nail look normal after fungus treatment? ⌄
The nail is not "cured" — it regrows. On the big toe that takes 9–12 months, on the others 4–6. The laser stops the fungus; appearance recovers as the healthy plate grows out.
? Do I need a test before treatment? ⌄
Yes, for onychomycosis. Nail trauma, nail psoriasis, onychogryphosis and the effects of tight shoes all look like fungus, and they are treated completely differently. A course started without a confirmed diagnosis is often wasted.

How long it takes
ParamedCalluses, hard skin and hyperkeratosis usually need one appointment, then maintenance every 6–10 weeks depending on load and footwear. An ingrown nail with a brace means 2–4 months of wear with check-ups every 4–6 weeks, until the plate grows out in the right shape. Onychomycosis is the longest: the laser course is typically 4–6 sessions 2–4 weeks apart, but a healthy big-toe nail takes 9–12 months to grow out, and the final result shows on that timescale rather than immediately.
Contraindications and limits
