Accent Prime vs SMAS lifting: what's the difference and who each suits
SMAS lifting isn't a single device but a group of methods working on the SMAS layer at 4.5 mm depth. Accent Prime is an RF device working in the dermis (1–4 mm). Different goals and different patients.

The comparison in brief
ParamedSMAS lifting is a collective term for methods that tighten the SMAS layer (Superficial Musculo-Aponeurotic System) at 4.5 mm. This is the layer plastic surgeons tighten during a facelift. Non-surgical options include HIFU (Ulthera, Doublo, Liftera), thread lifting and plasma lifting. They all work deeper than RF lifting and suit pronounced ptosis in patients 45+. Accent Prime is a different category: an RF + ultrasound device working in the dermis and subcutaneous layer (1–4 mm). It doesn't «lift» the SMAS, but quality-rejuvenates the skin and corrects the oval, nasolabial folds and jowls in younger patients (30–45) — or as maintenance after SMAS procedures.
When Accent Prime RF lifting is indicated
Paramed- ✅ Age-related changes of the face and neck
- ✅ Loss of firmness and elasticity
- ✅ Nasolabial folds and jowls
- ✅ Sagging facial oval
- ✅ Dull skin tone and lost radiance
- ✅ Fine lines around eyes and mouth
How we pick the protocol
ParamedAt the consultation the physician assesses skin condition, zones, goal and contraindications. If the goal is deep SMAS lifting, we recommend a HIFU device or thread lifting — we don't offer those. If the goal is rejuvenation, oval lifting, nasolabial folds, jowls or skin quality — we pick the optimal Accent Prime protocol: UltraFace for needle-free RF, Microplasma for fractional work, or a combination.
How the methods work
ParamedSMAS lifting is achievable through HIFU (focused ultrasound), threads (PDO/PLLA, lifting tissues mechanically) or surgery (classic facelift). All methods produce visible tissue tightening. Accent Prime uses UniPolar RF + Sharewave ultrasound — this is dermal heating, collagen stimulation and lymphatic drainage. Effect: skin quality improvement, light lifting, tone evening. These are different treatment layers: RF — dermis (1–4 mm); HIFU/threads/surgery — SMAS (4.5 mm and deeper).
- SMAS — the 4.5 mm musculo-aponeurotic layer
- SMAS-lifting methods: HIFU, threads, surgery
- Accent Prime — RF + ultrasound, 1–4 mm (dermis)
- SMAS lifting: for pronounced ptosis, 45+
- Accent: for skin quality and light correction, 30+
- Often combined: SMAS method + Accent maintenance
FAQ
Paramed? Do you offer SMAS lifting? ⌄
We don't perform classic SMAS lifting via HIFU (Ulthera, Doublo) or threads. Surgical facelift isn't our specialty either. We work on Accent Prime (Alma Lasers) — this is skin RF lifting, not SMAS.
? Who's better suited to SMAS lifting vs Accent? ⌄
SMAS lifting — for pronounced ptosis, loss of oval definition, sagging tissues in patients 45+. Accent Prime — for early age-related changes, lost tone, fine lines in patients 30–45, or as maintenance between SMAS sessions.
? Can an Accent course replace SMAS lifting? ⌄
Not fully. If the goal is deep lifting at the SMAS level, no. If the goal is skin quality, tone and a light pull on oval/jowls, an Accent course delivers a good result.
? Can both methods be combined? ⌄
Yes — this is a common strategy: one SMAS lifting procedure (e.g. HIFU) + a maintenance Accent Prime course once a year. This delivers deep and surface effects simultaneously.
